MedStar Health Institute for Quality and Safety to Host 6th Annual Crouse Patient Safety Lecture

Columbia, MD — MedStar Health Institute for Quality and Safety will host the 6th Annual Crouse Patient Safety Lecture on Oct. 2 at MedStar Franklin Square Medical Center. The event will mark the 20^th anniversary of the groundbreaking report, “To Err Is Human: Building a Safer Health System” which was issued in November 1999 by the U.S. Institute of Medicine and resulted in increased awareness of U.S. medical errors and a need for improved patient safety.

The panelists, three health policy and patient safety experts will provide a wide-ranging discussion on where we are today regarding patient safety. The panelists include: Lucian Leape, MD, adjunct professor of Health Policy at the Harvard School of Public Health; Carolyn M. Clancy, MD, Deputy Undersecretary for Health for Discovery, Education, and Affiliate Networks (DEAN) for the U.S. Department of Veterans Affairs; Martin “Marty” J. Hatlie, JD, co-director of the MedStar Institute for Quality and Safety and President & CEO of Project Patient Care.

“The report, ‘To Err is Human’ was groundbreaking in that it brought the issues of medical error to the forefront of national concern.” says Dr. David Mayer, executive director, MedStar Institute for Quality and Safety. “While we have come a long way in the last twenty years, we still have over 200,000 deaths a year in the U.S. due to unintentional but preventable hospital errors which is unacceptable. Improving patient safety requires a collaborative effort from all stakeholders, so I look forward to interactive discussions on this important topic with our esteemed panelists.”

Patient safety is a key initiative of MedStar Health and its mission to becoming a High Reliability Organization (HRO), which focuses on the elimination of preventable medical errors. HROs develop a culture in which staff feel empowered to identify safety or quality problems before harm occurs.

Panelist Biographies

• Lucian Leape, MD, is adjunct professor of Health Policy at the Harvard School of Public Health and a health policy analyst whose groundbreaking research has focused on patient safety and the quality of care. He is internationally recognized as the leader of the patient safety movement, beginning with the publication of his seminal article “Error in Medicine” in the Journal of the American Medical Association in 1994.
• Carolyn M. Clancy, MD, serves as the Deputy Undersecretary for Health for Discovery, Education, and Affiliate Networks (DEAN) for the U.S. Department of Veterans Affairs. Prior to her current position, she served as the deputy undersecretary for Health for Organizational Excellence overseeing veterans’ health administration, as well as 10 years as the director of the Agency for Healthcare Research and Quality. In 2018, Dr. Clancy was named, by Modern Healthcare magazine, as one of 2018’s 50 Most Influential Physician Executives and Leaders.
• Martin “Marty” J. Hatlie, JD, is co-director of the MedStar Institute for Quality and Safety. He also is CEO, Project Patient Care (PPC), a nonprofit organization that serves as the Chicago region’s safety and quality improvement coalition and is dedicated to using the voice of the patient to improve care. Mr. Hatlie also is the owner of Partnership for Patient Safety (p4ps®), which developed the acclaimed First Do No Harm® video series, now distributed in more than 40 countries.
