Coming soon: Guiding an Improved Dementia Experience

MedStar is excited to extend our dementia care services through the Guiding an Improved Dementia Experience (GUIDE) program. While working hard to accept the first patient in July 2025, a list of available current programs and specialists can be found here:

The GUIDE program will focus on comprehensive, coordinated dementia care to improve quality of life for people with dementia, reduce strain on their caregivers, and enable people with dementia to remain in their homes and communities.

The site will be updated as more information becomes available.

Eligibility

A beneficiary is eligible to receive services under the GUIDE Model if they meet the following criteria:

  • Has a diagnosis of dementia such as Alzheimer's Disease, as confirmed by their provider, AND
  • Enrolled in Medicare Parts A and B

 

A beneficiary cannot participate if they:

  • Are a long-term nursing home resident, OR
  • Have a Medicare hospice benefit, OR
  • Have Medicare Advantage or a special needs plan like PACE, OR
  • Are enrolled with another GUIDE Participant
