Guiding an Improved Dementia Experience

MedStar Health is excited to extend our dementia care services through the Guiding an Improved Dementia Experience (GUIDE) program.

GUIDE is a new Medicare program designed to enhance the lives of people with dementia and their caregivers. Program goals are to help people with dementia remain safe in their homes for longer, improve care coordination, provide access to resources, and help families and caregivers through training, education, and support services. Persons living with dementia (PLWD) enrolled in the GUIDE model who have moderate or severe dementia will also have an in-home visit. They may be eligible for limited in-home or adult day care respite service benefits.

We are currently accepting patients from Central Maryland, Washington D.C., and Northern Virginia

Eligibility

A beneficiary is eligible to receive services under the GUIDE Model if they meet the following criteria:

  • Has a diagnosis of dementia such as Alzheimer's Disease, as confirmed by their provider, AND
  • Enrolled in Medicare Parts A and B as primary insurer

 

A beneficiary cannot participate if they:

  • Are a long-term nursing home resident, OR
  • Have a Medicare hospice benefit, OR
  • Have Medicare Advantage or a special needs plan like PACE, OR
  • Are enrolled with another GUIDE Participant

Enrollment process

  1. Referral: Based on your visit, your provider may make a referral to the GUIDE program.
  2. Review: The GUIDE team will review your information to see if you meet Medicare’s requirements for the program.
  3. Virtual/In-Person Meeting: The GUIDE team will schedule a time to meet with you and your family/caregiver to validate your information and ask questions that help us understand how and what you and your caregivers are feeling and doing in your daily life.
  4. Medicare Review: Your information is securely submitted to Medicare for review and to determine if you qualify.  This may take up to 3 weeks for Medicare to let us know if you qualify for GUIDE.
  5. Notification: We will let you know about Medicare’s decision by either a phone call and or a beneficiary notification letter that will be mailed to you/your caregiver. 
  6. Support Begins: If Medicare accepts you into the GUIDE program, a member of the GUIDE team will schedule time with you/your family/caregiver to review the program and complete a home safety assessment.
