The MedStar Health—Georgetown University Hospital Fellowship Program in Neonatal—Perinatal Medicine

MedStar Health - Georgetown University PartnershipThis fellowship program is fully accredited by the Accreditation Council for Graduate Medical Education (ACGME) and meets the requirements of the American Academy of Pediatrics Subspecialty Board in Neonatal/Perinatal Medicine. The fellowship is a part of the Division of Neonatology and Neonatal Intensive Care Unit (NICU) Services.

This three-year program generally accepts two fellows per year and is designed to help them achieve:

  • Professional competence in the clinical care of the critically ill newborn

  • Skill in the design and conduct of clinical and basic science research

  • Excellence in teaching

Why train here

  • Gain clinical competence and develop expertise in providing hands-on, high-quality, evidence-based clinical care to critically ill and convalescent newborns in our Level IV maternal and neonatal regional referral center NICU that includes a high-risk inborn service and treatment for infants with a wide variety of medical and/or surgical diagnoses cared for by our neonatology team with support as needed from pediatric surgery and pediatric medical and surgical subspecialty services. We offer coordinated care from before the time of delivery, with prenatal consultation for high-risk pregnancies (with collaboration between neonatologists, maternal fetal medicine experts and pediatric subspecialists as needed), through an acute-care NICU stay, to a step-down NICU service, and continuing post-discharge with our neonatologist-led high-risk neonatal follow up clinic as well as neurodevelopmental follow up care.   • Gain proficiency stabilizing newborns at outside hospitals and transferring them to our NICU via air and ground transport as the transport team lead. Our fellow-led 24/7 helicopter and ground neonatal transport service accepts referrals from DC, Maryland, Virginia, and West Virginia as well as from overseas.   • Access to state-of-the-art equipment

    • Point-of-care ultrasound (POCUS)

    • Video laryngoscopy for neonatal intubation

    • Near Infrared Spectroscopy (NIRS)

    • Breast milk analyzer for nutritional analysis of human milk allowing for personalized precision nutrition

    • Amplitude-integrated EEG and video EEG

    • Whole-body cooling for neonatal hypoxic-ischemic encephalopathy

    • ECMO with cannulation in the NICU and management by our neonatologists, our fellows, and our nursing ECMO specialists

    • Multiple modes of respiratory support including high frequency Jet ventilation (HFJV); bubble, Draeger, and Ram CPAP or NIPPV; Vapotherm; and high-flow nasal cannula

  • Gain skills in designing and conducting scholarly projects (in clinical, basic science, translational, educational, global health, and quality improvement research) with opportunities to present at regional and national meetings and publication of research findings.

    • The Georgetown medical campus is home to a rich scholarly and research community spanning basic and behavioral biomedical sciences, medical humanities, clinical research centers, medical education research, global health and health policy, and medical ethics. Fellows have access to resources from three overlapping organizations: the MedStar Health Research Institute (MHRI), Georgetown University Medical Center (GUMC), and the Georgetown-Howard Universities Center for Clinical and Translational Science (GHUCCTS).     • Fellow research may occur with mentors in the Division of Neonatology or with faculty in other departments at MedStar Georgetown University Hospital (MGUH), the MedStar Health network, Georgetown University Medical Center (GUMC), or the nearby National Institutes of Health

    • Presentations at regional and national conferences are encouraged and support provided

  • Research funding is available through a number of means including the support from the MedStar Georgetown Neonatology Fellows Research Endowment Fund, which provides financial grants specifically to support fellows’ research activities within the MedStar Health Georgetown University Neonatal-Perinatal Medicine Fellowship Program

  • Participate in a robust, evidence-based educational conference schedule with 4-5 hours a week of dedicated conference time including a POCUS curriculum with bedside teaching, procedure workshops and simulations, dedicated board review, case conferences, perinatal conferences with OB and NICU, EID and health care disparities education, wellness curriculum, and a professional development series. Training in biostatistics, genomics, bioethics, and ECMO are included components of our fellows’ education.

  • Gain insight into administrative aspects of running a NICU through participation in quality improvement projects and multidisciplinary committees. Train for future leadership roles in the field through participation in the NICU Multidisciplinary Administration Committee and Quality Improvement Committee along with other leadership opportunities.

  • Participate in one of the region’s only neonatologist-led high-risk neonatal follow-up clinics

  • Provide family-centered, personalized care with a multidisciplinary team including occupational, physical and speech therapists, nutritionists, social worker, case manager, psychologist, family support specialist, and chaplain services for a wide range of faiths to support our patients and families. Our fellows learn to care for newborns consistent with Georgetown’s tradition of “Cura Personalis,” or care for the whole person.

Curriculum

This three-year program includes in-depth clinical training as well as a robust schedule of didactic lectures and conferences plus involvement in research (either clinical, educational, translational, or basic science-based), which is intended to lead to presentation at a national or international meeting and publication in a peer-reviewed journal, as well as participation in one or more quality improvement efforts.

Year 1

The first year of training involves approximately five months of clinical time in the NICU working closely with the attending neonatologist. Fellows gain experience in the assessment of high-risk pregnancies, antenatal consults, delivery room care and resuscitation, transportation of the critically ill neonate by ambulance or helicopter, all aspects of care of extremely low birth weight infants, and high technology therapy such as high frequency jet ventilation, extracorporeal membrane oxygenation (ECMO), nitric oxide therapy, and whole body cooling for neonatal hypoxic ischemic encephalopathy.

Fellows supervise and perform a variety of procedures, including umbilical venous and arterial catheterization insertion of peripheral arterial catheters, percutaneous central venous catheters, and thoracostomy tubes. They supervise residents and medical students, and work with nurse practitioners and physician assistance during their clinical rotations.

During a two-week rotation in the Maternal Fetal Medicine Service, fellows learn to assess high-risk pregnancies via antenatal testing, genetic amniocentesis, and fetal ultrasonography. Fellows provide consultations for high-risk obstetrical patients, discuss management of high-risk deliveries and fetal monitoring, and participate in perinatal-neonatal conferences throughout the three years of fellowship. They attend High-Risk Follow-up and Developmental Outpatient clinic sessions, in order to learn the outpatient management of NICU graduates.

The balance of their time in the first year is spent on research and scholarly activity, learning the essentials of study design and statistics. They are encouraged to develop their own projects under the guidance of the research mentor. They participate in all departmental and divisional teaching activities. They also complete Neonatal Resuscitation Program Instructor Training.

Year 2

In the second year of training, fellows further refine their clinical and supervisory skills. Additional experiences in the second year of training include:

  1. A pediatric cardiology and cardiovascular surgery rotation at the Children’s National Hospital CICU
  2. A rotation at MedStar Washington Hospital Center NICU (level IIIB NICU and the delivery center for Children’s National Hospital)
  3. A “community neonatology” experience at Sibley Memorial Hospital NICU (a high-volume delivery center and level II Special Care Nursery covered by Georgetown neonatology attendings).

There is increased emphasis on research during the second year with time spent in laboratory or clinical research. Close involvement with research mentors during this time is essential to ensure appropriate progress toward the goal of a completed research project, leading to a peer-reviewed publication by the end of the period of training.


Year 3

In the third year of training, fellows further refine their clinical, supervisory, and leadership skills. The third year is largely devoted to the completion of scholarly activity. Fellows have increasing clinical responsibility and autonomy as their time in training progresses, culminating in a fellow’s final clinical rotation in which the fellow becomes the “acting attending” of the acute-care service.

Call schedule

Night and weekend calls continue throughout the three-year period, are in-house, and average to one in six nights. The program conforms to the ACGME requirements for duty hours.

Vacation

Fellows are allowed four weeks of vacation in each year of training.

Conferences and lectures

The following didactic lectures, conferences, and other events are also a regular part of the fellowship program.


  • Neonatology Boot Camp

    This series of lectures in the fellowship’s initial weeks of the academic year covers the basic topics of neonatology: respiratory diseases and principles of conventional, high-frequency, and non-invasive ventilation; cardiorespiratory support; fluid and nutrition support; hematology; infectious disease; neonatal neurology; and neonatal transport. Boot camp also incorporates transport simulations and interactive procedure workshops. First-year fellows also attend a two-day regional neonatology boot camp in collaboration with other first-year neonatology fellows from across the region for additional hands-on training.

  • ECMO Training Course

    This annual two-day course includes lectures on membrane physiology, gas exchange, indications and contraindications of ECMO, patient management, troubleshooting, and practical hands-on training in neonatal ECMO.

  • Weekly Division Educational Conferences

    Conferences for the entire Division of Neonatology are held every Wednesday afternoon. Presentations are given by fellows, division faculty, or visiting faculty. These conference topics include:

    • Division of Neonatology Grand Rounds: Invited faculty speakers from within or outside of the division.

    • Morbidity and Mortality Conference: Presentation of detailed chart review and evidence-based discussion of management of patient(s) who expired or had serious complications. Involvement of all subspecialists in the patients’ medical/surgical care is encouraged. Includes a presentation of pathology/autopsy results by the Department of Pathology as applicable.

    • Clinical Management: Discussion of a clinical management topic based on evidence, or discussion of controversies in clinical care.

    • Research Discussion: Fellows or faculty present research progress including seeking suggestions and feedback. Some of these sessions are fellows’ Scholarship Oversight Committee presentations.

    • Journal Club: Discussion of a recent article, with review of background, and detailed analysis of methodology, results, and implications of the work. 

    • Physiology or Pathophysiology Presentation: Presentation on an area of fetal-neonatal physiology or pathophysiology

  • Clinical Case Conference

    Held about once a month following the weekly Division Education Conference on Wednesday afternoon, Division of Neonatology faculty and fellows gather for a clinical discussion of an interesting or difficult NICU case with an opportunity for open discussion.

  • Simulation/Procedure Sessions

    Held about quarterly, faculty and fellows gather for simulation of procedures such as exchange transfusion, cardioversion, aEEG placement, pericardiocentesis, and chest tube insertion, among others.

  • POCUS (Point of Care Ultrasound) Series

    Held once a month, this series teaches fellows skills of POCUS (point of care ultrasound) in neonatology with lectures in conjunction with hands-on experience. 

  • EID and Healthcare Disparities Series

    Held several times a year, these presentations on Equity, Inclusion, and Diversity cover topics such as bias and microaggression as well as presentations addressing Healthcare Disparities

  • Physiology/Neonatology Board Review Sessions

    Held about three times a month, usually following the weekly Division Education Conference on Wednesday afternoon, fellows gather for an interactive discussion of an area of fetal-neonatal physiology, biochemistry, or developmental biology. An attending or fellow provides/ suggests reading material in advance, reviews material with fellows, and discusses board-type questions

  • Wellness Curriculum

    Fellows meet with NICU psychologist, Dr. Saroop, about every six weeks for discussions/ activities to address wellness and resiliency. Additional social activities both at the hospital and out in the greater DC region such as painting, attending a performing arts event, or cooking supplement this wellness curriculum. 

  • Professional Development Series

    Held a few times a year, this series includes presentations on topics such as job negotiation, completing a death certificate, informed consent, recontextualizing the difficult parent, financial management, etc.

  • Obstetrics/Neonatology Perinatal Conference

    These are joint presentations between an obstetrics fellow and a neonatology fellow, held bimonthly on a Tuesday or Wednesday morning providing a forum for discussion of a clinical case along with review of a topic of common interest to Neonatal-Perinatal Medicine and Maternal Fetal Medicine.

  • NICU Mock codes

    These multidisciplinary mock codes involving residents, fellows, NICU RNs, and RTs, among others, are led by NICU faculty and fellows once or twice a month

  • Department of Pediatrics Grand Rounds

    Held every Friday morning, these lectures feature invited local, national, and international experts to present on topics relevant to pediatrics.

  • MedStar Washington Hospital Center Visiting Lecturer Series in Perinatal Pediatrics

    Held once a month on a virtual platform, faculty speakers from MedStar Washington Hospital Center, Children’s National Hospital, or outside invited speakers present topics relevant to neonatology.

  • Neonatal Intake Rounds

    A monthly presentation by a pediatrics resident of an interesting NICU case with review of relevant literature.

  • Neonatal Core Lecture for Pediatrics Residents

    A review of a core neonatology topic for residents in the pediatric residency program, medical students on their pediatrics rotation, and neonatology fellows. Topics are presented either by a Division of Neonatology faculty member or by a Neonatal-Perinatal Medicine fellow jointly with a neonatology faculty member and occur about every 6 weeks.

  • National Neonatology Webinars

    Fellows are encouraged to participate in a variety of webinars including the Vermont Oxford Network (VON) Webinar Series, the Cochrane Neonatal Webinar Series, the Organization of Neonatal-Perinatal Medicine Program Directors (ONTPD) Online with the Experts Series, the Newborn Brain society (NBS) Webinar Series, the National Neonatal POCUS (Point of Care Ultrasound) Collaborative Webinar Series, the Genomic Network Webinar Series, and National Institute for Children’s Health Quality (NICHQ) webinars.

  • Dahlgren Memorial Library Research Lectures and Webinars

    The Georgetown University School of Medicine Library (Dahlgren Memorial Library) runs regular conferences on everything from searching the medical literature to developing survey tools and performing database searches to using data entry and storage systems to perform clinical research.

  • NIH Introduction to the Principles and Practice of Clinical Research

    First-year fellows participate in this online course about the principles of clinical research and biostatistics. It runs from October through February.

  • Discharge Planning Rounds

    On-service fellows participate in these weekly multidisciplinary meetings, which include nursing, case management, social work, therapy (occupational, physical, and speech), and neonatal clinic staff. The meetings allow fellows to consider the different aspects of safely transitioning complex NICU patients to home.

  • Other monthly meetings

    Fellows gain administrative experience by participating in monthly meetings of the Multidisciplinary NICU Administrative Committee and the Quality Improvement Committee. Fellows are also encouraged to become involved in other hospital or NICU committees, including the NICU Transport Committee, the NICU Patient and Family Advisory Council for Quality and Safety, the Postpartum Depression Screening in the NICU Committee, the NICU Discharge Planning Committee, the NICU Bereavement Committee, among others.

Research

All division members are actively involved in clinical or laboratory research. Research opportunities are not restricted to the Division of Neonatology, and the fellows are encouraged to explore avenues of research with any faculty member at MedStar Health Georgetown University Hospital, Georgetown University Medical Center, or the National Institutes of Health.

A Scholarship Oversight Committee (composed of the research mentor and two or more additional individuals, at least one of whom is outside the Division of Neonatology) is appointed by the end of the first year and meets quarterly thereafter to monitor the fellow’s progress. The research mentor guides the fellow in developing the research hypotheses, designing the study, and developing the protocol.

Fellows are provided with 20 hours of free biostatistics help through the MedStar Health Research Institute for assistance with data analysis. Faculty mentors provide assistance with manuscript preparation. Fellows can apply for research funding through the Neonatology Fellows Research Endowment Fund, a fund available to all Georgetown Neonatal-Perinatal Medicine Fellows to support fellows’ research projects, travel-related expenses for research conferences and educational priorities aligned with the fellows’ academic career. Fellows are encouraged to present their work at national and international meetings.
